In consideration of my child(ren)’s attendance and participation in the martial arts training offered by Mastery Martial Arts, I the parent/guardian acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks.
I further relieve Mastery Martial Arts, its management, assigned staff and fellow students, and/or agents of the above from any liability resulting from loss, whether personal belongings or bodily injury on the premises leased or owned by Mastery Martial Arts.
I also hereby state, that my child(ren) is/are physically fit to take the prescribed course of instruction and do so of my own free will. I give permission to be added to the email list.
